WHAT TO BRING:

• In the case of a company, the certificate of incorporation, as well as a list reflecting the ID number, the full names and surname, residential, postal and work address of each director, manager and secretary, must be attached.
• In the case of a close corporation the CK1or CK2 must be attached.
• In the case of a partnership a separate schedule reflecting the full name and surname, identity number and address of each partner must be attached.
• In the case of a sole proprietor a copy of the owners ID must be attached
• In the case of a restaurant or café keeper a menu must be attached.

FEES:

Application fee: R420.00 (Sale or supply of meals or perishable foodstuffs)
Must renew after 12 months – Renewal Fee: R306.00.
Application fee: R3 780.00 (Provision of certain types of health facilities or entertainment, Adult premises)
Must renew after 12 months – Renewal Fee: R2 700.00.

• Forms must be filled in Duplicate (x2)
• Necessary documentation must be attached
• The business premises must be inspected by Health & Fire inspectors (They must give reports back to this section)

The business premises must be zoned for the type of business that will be conducted.
