Where applicable, the following forms or documents must be complete by the applicant:

• Form A: Particulars of the deceased
• Form B: To be completed by the family doctor
• Form C: To be completed by a medical doctor (other than the family doctor)
• Form D: To be completed in cases of unnatural death
• Form E: To be completed by a medical referee appointed to a panel by the City of Tshwane
• Burial order (from the undertaker)
• Statement of payment
• Reopening request (book)
• Tombstone application form

Applicants are advised that the booking will only be confirmed once all the required documentation is correctly completed and returned to cemetery office together with the proof of payment. Payment is usually made at the nearest City of Tshwane pay point (excluding drive-through pay points).
